Stress Testing During Cardiac Catheterization: Exercise and Pacing Tachycardia

چکیده

Patients with significant heart disease may have entirely normal hemodynamics when assessed in the resting state during cardiac catheterization. Because most cardiac symptoms are precipitated by exertion or some other stress, however, it also may be important to assess hemodynamic performance during some form of stress such as muscular exercise, pharmacologic intervention (e.g., dobutamine infusion), or pacing-induced tachycardia. Such an evaluation enables the physician to assess the and the relationship (if any) between specific symptoms and hemodynamic impairment. Physiologic information so obtained is often valuable in prescribing specific medical therapy, selecting patients for corrective cardiac surgery, and estimating prognosis. cardiovascular reserve

The role of stress test for predicting genetic mutations and future cardiac events in asymptomatic relatives of catecholaminergic polymorphic ventricular tachycardia probands.

AIMS Catecholaminergic polymorphic ventricular tachycardia (CPVT) is an inherited arrhythmic disorder with a highly malignant clinical course. Exercise-stress test is the first-line approach to diagnose suspected individuals. We sought to elucidate the value of exercise-stress test for predicting mutations and future cardiac events in CPVT-family relatives. METHODS AND RESULTS The present stu...

Ineffectiveness of glucose, potassium, and insulin infusion during pacing stress in chronic ischemic heart disease.

Eight patients with coronary artery disease and positive exercise electrocardiograms undergoing cardiac catheterization were subjected to periods of atrial pacing-induced tachycardia at identical rates, before and during a glucose, potassium, and insulin (GKI) infusion. Steady state elevation of glucose and insulin was obtained prior to repeating the atrial pacing test in the GKI state. With ea...

Life threatening ventricular tachycardias in late survivors of surgically corrected tetralogy of Fallot.

Electrophysiological tests were performed in three patients with surgically corrected tetralogy of Fallot (mean age at evaluation 25 years, mean age at surgical correction 4 years) who had had either a cardiac arrest or transient neurological disturbances (presyncope, syncope) associated with ventricular arrhythmias. All three patients had an excellent haemodynamic result from surgery as judged...

Glucose extraction by the human myocardium during pacing stress.

Glucose extraction by ischemic human mvocardium was investigated at the time of diagnostic cardiac catheterization in 27 subjects who had fasted overnight. Paired arterial and coronary sinus blood samples, obtained before and during coronary sinus pacing, were analyzed for glucose and lactate. Pacing to a rate 50 to 70% greater than control or to the development of chest pain induced no signifi...

Evaluation of children with ventricular arrhythmias.

Ventricular arrhythmias are rare in childhood but may be associated with syncope and sudden death. This report describes 8 children with ventricular arrhythmias, 6 of whom suffered syncopal episodes. Ventricular tachycardia was documented in 5. One boy died suddenly. Complete cardiac investigation was carried out with exercise testing, Holter monitoring, echocardiography, cardiac catheterizatio...
